When Size matters.

Shapely and proportionate breasts add a lot of confidence to a woman's personality. It is not surprising that Breast augmentation is one of the most requested cosmetic surgery procedures the world over. Aeound 3,00,000 women underwent this procedure in 2018 in USA alone. It is important that it is performed for the right reasons though, not under peer pressure, or demands of other individuals. The benefits of the procedure can be the following-
1. Breast Augmentation increases the size of breasts.
2. The procedure can restore volume loss after pregnancy or massive weight loss.
3. It can improve the shape and symmetry in certain conditions like tuberous breasts.
4. It increases the fullness and projection of breasts.
5. Improves the balance of breast and hip contour.
6. Enhance your self-image and self confidence.

There are certain pre-requisites for undergoing the procedure. Firstly, the physical growth of the breasts should be complete. For this it is recommended to be performed after achieving 22 years of age. The lady must be physically fit and not actively breast feeding or pregnant at the time of the procedure.

Breast augmentation is commonly performed with either Implants or Fat grafting.
There are various types of Implants which can be opted for. Currently the 4th and 5th generation silicon implants are commonly used in India. They have good tenacity and contain highly cohesive silicon gel with double barrier protection, which make them more resistant to trauma and gel bleed and long-lasting. They can be smooth, microtextured or nanotextured. They can be round or tear drop shaped. They are available in various sizes which can be selected based on the patient's body type and preference. Usually a 200cc implant leads to increase in 1 cup size.  During the consultation, with the use of sizers, we can estimate the potential increase in projection which can be achieved.
The implants are usually inserted through incisions taken in skin creases and are well concealed.
This being a day care surgery has minimal downtime and the patient can resume daily activities within 3-4 days.

Though the procedure is quite safe, there are certain risks that one needs to be aware of. These include haematoma, seroma, infections in the early stage. And capsular contracture in the later stage.

Breast augmentation does not interfere with mammograms and allow regular screening. Also the regular incisions do not intersect the ducts and the implants are placed underneath the glands, so they do not interefere with breast feeding.

In Cases of deflated breasts post pregnancy or weight loss, one can opt for something called the 'Augmentation Mastopexy', which involves Breast Lift and Augmentation with Implants.

In certain cases where one does not want to use implants we can achieve good results even with Fat grafting. In this scenario, we usually harvest fat from areas of excess like belly and thighs with the help of tiny cannulas and inject this fat into the breast. However there is a limit to the amount of fat that can be injected in one go and this usually requires at least 2-3 stages to achieve the desired results. With the use of growth factors we can achieve better viability and quality of fat grafts.

Breast augmentation is a highly individualised procedure which needs detailed evaluation and counselling. One should discuss all the pros and cons with their surgeon before opting for it to achieve the best results.

Years ago, I received a phone call for an interview as surgical registrar at a reputed hospital. I was in a fix, as I was stranded with a 9 month old infant at home for that week, my in-laws were out of station and my husband was busy at work.
